Grace Life Co-op requires each family to do the following before group membership is approved:
1. Complete this application.
2. Sign the Liability Waiver and Sickness Waivers.
3. Meet an administrator (for new families).
4. Pay applicable fees.

Please note that membership for the 2026-2027 is not guaranteed, but applications are kept on file for one year. Once returning members have renewed/registered, available space will be evaluated and you will be contaced accordingly. Thank you!

Additional Questions

1. Please list any allergies/medical conditions/special needs you or your children have of which we need to be aware at Grace Life activities. Please specify for which family member this applies. If none, please type N/A. 2. Please list anything that you would like to communicate to the teacher(s) that may help enhance your child's learning environment. For example, if your child has Dyslexia, Dysgraphia, ADHD, Autism, Asperger's, etc. please let us know so that we may attempt to work with, and not accidentally against, your child. Please keep in mind that we are not a school. We are families coming together to create unique learning experiences for our children.

If there is nothing to list for your family, please type N/A. 3. Please list TWO emergency contacts to include name, relation to the children enrolled, and a phone number for each contact. 4. In the event of an emergency, every possible measure will be taken to reach the parent(s)/guardian(s) and/or the emergency contacts listed.

In the case that the parent(s) and/or contacts cannot be reached, I give permission for the agents directly involved with this co-op to initiate seeking medical treatment deemed immediately necessary.

By typing my name below, I am agreeing with this policy. 5. I understand that this is a co-op (cooperative), meaning that each family contributes to its success. If I am not a teacher, director, or administrator of co-op, I, or my spouse, will sign up for a parent duty and will fulfill our obligation to this parent duty.

In addition, I have read and understood the Liability and Sickness Waivers and I take full responsibility for reading the information within these documents.

These documents are found on the top navigation bar on the home page of the Grace Life Co-op website.

By typing my name, I am stating that I have read and understand each of these policies and agree for myself and my child(ren) to be held to them. 7. I understand that there is a Registration Fee of $25/family/year and a $20 Director's Fee.

Families will also be asked to give $25 per semester (total $50/year) to our host church as a thank you for the use of their facilities. In the event of an emergency (unforeseen maintenance repair), we may request/require a small additional charge per family. *If your child(ren) intentionally or unintentionally damage(s) the property of our host church, your family will be solely responsible for the monetary cost to repair the damage.*

The cost(s) of each class is/are set by the teachers for his/her individual class(es) to include supply fees and monthly class fees, which may be paid diretly to the teachers via their preferred payment methods. Once your application is approved, you will have access to each class description, which lists the preferred methods of payment for each teacher.

Class fees are due the first Monday of each month (for a total of 8 payments). 29 Mondays are included each year on our schedule.

I agree that participation in Grace Life Co-op is voluntary and indicates that I will voluntarily drop my child(ren) off in order to attend the classes.

I understand that I, my spouse, or guardian responsible for my child(ren) may stay on the premises during class instruction as long as I/we do not disrupt any classes.

I have read and signed the Grace Life Liability Waiver and I agree not to hold Grace Life, our host church, or any member thereof responsible for any accidental occurence in my absence or presence.
